I came across a Steam forum that highlighted how Fallout can sometimes get the details of the graphics card wrong:

Download DLL to force Fallout3 to "see" an NVIDIA Geforce card.
It stopped the crash on the loading screen, but Fallout3 ran very laggy.

To get Fallout3 to "see" the AMD graphics card on my laptop, I went into my Device Manager in Windows, and under Display Adapters I updated the drivers for both (Intel integrated graphics was already up to date, the AMD dedicated graphics needed an update).

So I don’t have a concrete fix for you unfortunately, but I do have suggestions:
1. Go into your Device Manager (right click the windows icon) and see what you have listed under ‘Display Adapters’
2. On the Fallout3 game launcher, select ‘Options’ and see what graphics Fallout is looking to use to render the game
3. If it is looking at an Intel integrated graphics, try to get it to point to your dedicated graphics card instead by updating it’s drivers
If it is correctly looking at your dedicated graphics card, see if there are newer drivers and make it up to date
4. If none of the above works, use the DLL file method described in the Steam forum (or check out the other nexusmod links above)

Fallout 3 just got updated on Steam to fix Games for Windows Live related crashing issues, which has fixed the primary vanilla stability issues for the Steam version of the game. It has unfortunately broke mods, including FOSE (Fallout Script Extender), any mods that require FOSE, and installers that check the game version (such as TTW). Workarounds are in development. I also briefly go over my thoughts on this hinting toward a Fallout 3 Remake or Remaster coming soon.

On PC open console (~ key) and type player.kill
On consoles kill yourself in other ways, for example by jumping from high places.
